Resource Navigation & Community Connections
Finding the right resource is often more complicated than simply locating a phone number or website.
Little Light of Mine provides individualized resource navigation designed to help clients understand their options, identify appropriate outside services, prepare for referrals when needed, and move forward with the next appropriate step.

More Than a Resource List
Our role is not simply to hand someone a list of organizations and expect them to figure out the rest.
Individualized resource navigation may include:
Identifying services that match the client’s specific needs
Researching available programs and options
Reviewing eligibility requirements
Determining whether a referral or assessment is required
Helping organize information or documents needed for the next step
Assisting with initial calls or communication when appropriate
Helping clients prepare for intake, appointments, or assessments
Following up when additional assistance is needed
Identifying alternative options when the first resource does not work
Referral Support
Some outside organizations require a professional referral, assessment, application, appointment, or specific eligibility criteria before services can begin.
When a referral is necessary, Little Light of Mine can assist the client with understanding the referral process, identifying an appropriate resource, and coordinating next steps within the scope of our services.
Acceptance, eligibility, placement, and availability are ultimately determined by the receiving organization.
Community Partnerships & Professional Connections
Little Light of Mine works to develop and maintain relationships with community organizations, behavioral health providers, treatment programs, housing resources, workforce programs, and other community-based services.
These professional connections allow us to better understand available programs and provide more informed resource navigation based on each client’s individual circumstances.
Little Light of Mine is not affiliated with every organization to which a client may be connected, and a referral does not guarantee acceptance or services.
We Do Not Stop at the First Option
Sometimes the first resource is full, unavailable, does not accept the client’s funding source, has eligibility requirements the client does not meet, or simply is not the right fit.
When that happens, the resource-navigation process does not automatically end.
Within the scope of the client’s service, Little Light of Mine will continue exploring appropriate alternatives and available options rather than relying on a single referral.
Our goal is to help the client move toward a workable next step.
Individualized Resource Navigation
Every person’s circumstances are different.
A resource that is appropriate for one client may not be appropriate for another. Resource-navigation sessions allow us to consider factors such as:
Immediate needs
Recovery goals
Behavioral health needs
Location
Financial circumstances
Insurance or payment options
Eligibility requirements
Family circumstances
Employment
Housing
Transportation needs
Existing services and supports
Because Little Light of Mine provides transportation assistance directly when available, transportation needs can also be considered as part of the client’s overall support plan.
